Maternity document upload

Thank you for completing a maternity new client form.

Please send in copies or upload any documents you think may be relevant to your baby’s injury. These may include, if you have them:

  • Your obstetric and/or baby’s neonatal medical records
  • Letters you have received (such as duty of candour letters)
  • Complaint correspondence
  • Reports (such as HSIB investigation reports, Serious Incident Reports (SIR) or any reports or investigations which the trust or NHS Resolution has provided you with).

You may have all, some or none of the above documents. If you have electronic copies of these documents in Word or PDF format they can be uploaded through the portal below. Alternatively, if you prefer, please send them to us by post at the address shown.
