Real stories, real change
The report features personal stories, like that of June and Tricia, whose father died following avoidable medical harm. With Avma’s support during the inquest process, they were able to seek answers and accountability.
She [Avma’s adviser] was a refreshing voice during a very emotional time. Her knowledge made a huge difference.

What’s next for Avma?
In 2025–26, we will continue to build on this year’s achievements. We plan to pilot the Harmed Patient Pathway with healthcare providers, expand our reach through a new website, and continue supporting the All-Party Parliamentary Group on Patient Safety as its Secretariat.
