Reporting a patient safety incident


If patients are put at risk in a clinical setting, it’s important to report the incident to the right professional body.

Reporting to a professional body

If you or someone you know has experienced an incident in a health and care setting which you feel has put yourself or them at risk, there are different professional bodies you can contact to report this. These bodies cover all elements of health and social care, and there are different places to go depending on where you are in the UK.

Where to report an incident in England

Adverse effects of medicines and devices

If you have experienced adverse effects from medicines or medical devices, you can report this to the Medicines and Healthcare Products Regulatory Agency (MHRA) under their Yellow Card scheme.

Incident with your healthcare provider

If you have an experienced an incident with your local healthcare provider (including social care), you can provide feedback to the Care Quality Commission (CQC). It cannot usually investigate individual complaints, but can use feedback to inform their regulatory and inspection roles and will investigate a complaint where there is a potential systemic issue with the healthcare provided.

A health and safety incident
